Why Not Just Use a Faster Molecular Test?
A molecular test can tell you TB is present. A culture can tell you whether the drugs will work.
Molecular tests such as GeneXpert MTB/RIF can detect TB DNA within hours, making them significantly faster than culture methods. So why does culture remain important?
A molecular test detects the presence of the bacterium, while culture grows live bacteria that can be tested directly against a panel of drugs. This is what makes drug-susceptibility testing possible: determining whether the specific strain infecting a particular patient will respond to first-line medicines or has already developed resistance.

Why Drug-Susceptibility Testing Is the Whole Point
This is why culture cannot simply be replaced. What Culture Media Actually Are?
A culture medium is, in effect, food for bacteria. The long-standing gold standard for TB is Lowenstein-Jensen medium, developed in the mid-twentieth century and still in routine use worldwide. It contains a whole-egg suspension for lipids and proteins, glycerol as a carbon source, and malachite green, a dye that suppresses faster-growing contaminant bacteria and fungi so the slow MTB is not overwhelmed. Because the medium is essentially nutrition, one way to accelerate growth is to enrich it, adding targeted nutrients so the organism has less metabolic work to do, which is the principle behind recent faster-culture research.
The Contamination Problem
The prolonged culture process creates a second challenge. The longer a sample remains incubating, the greater the opportunity for other microorganisms to contaminate it, potentially forcing the test to be repeated or discarded.
Beyond this, tuberculosis research requires advanced, high-containment BSL-3 laboratories with specialised ventilation systems that maintain inward airflow. Since Mycobacterium tuberculosis is airborne, this infrastructure is expensive to build and maintain.
Shortening the culture window reduces both contamination risk and the pressure on limited containment capacity. This is why speed and reliability are connected rather than separate goals.
